One time $75 enrollment fee, maximum $150 per household. Members age 0-17 without an adult member are $55/mo.
HOW TO JOIN
Follow the steps below and call or email us if you have any questions about membership or registration.
1. Register
Click the link above to register as a new patient. The form will take around 15 minutes to complete and you will be asked some questions about your medical history. If there is anything that you don’t know offhand you can leave it blank and we will add any necessary information from your past medical records. We recommend using a computer, not an ipad or phone to register.
2. Request Records
When you complete the registration, please use the form below to request your medical records from your current primary care office.
3. Schedule An Appointment
As soon as your registration is accepted you are free to reach out to us to schedule your first appointment. We look forward to caring for you.
